I, the undersigned, hereby grant and assign to the Pacific Telecommunications Council (“PTC”) the full and irrevocable right to use my name, audio recordings of me, and pictures and/or video recordings or likenesses of me, or in which I may be included in whole or in part (“Pictures, Audio, and Video”), and any reproductions or adaptations thereof in any medium now known or hereafter known, taken by or on behalf of PTC during the PTC’25 Conference, January 19-22, 2025, or at any related event or location.

This permission includes the right to publish, distribute, and/or sell the above-identified material (“Material”) in any and all forms, including, but not limited to, print and digital formats, such as publications, websites, digital libraries, videocassettes, cablecasts, broadcasts, laserdiscs, multimedia, or other media formats, now or hereafter known.

I further grant PTC all rights, including but not limited to copyrights in and to the Pictures, Audio, and Video, which PTC may secure exclusively in its name or that of any designee. PTC is granted the perpetual and irrevocable right, but not the obligation, to use and reuse my name, Pictures, Audio, and Video for non-defamatory purposes, including but not limited to advertising, promotion, fundraising, or trade, in any medium now known or hereafter known.

I warrant that I am the sole owner of the rights referred to herein, and that the Material is free from any libelous, infringing, or unlawful matter. In the event that any elements used in the Material contain third-party work, I have secured the necessary permissions and will provide them to PTC. If a citation to a copyrighted work is required, I have obtained the correct wording and included it in the text. I agree to indemnify PTC against any loss, injury, or damage resulting from any breach of these warranties, including legal costs or expenses.

I hereby release and discharge PTC, as well as all sponsors and organizers of the Publication, from any and all liability arising from my inclusion in the Material or the use of Pictures, Audio, and Video, as permitted herein. This release includes, but is not limited to, all claims arising out of rights granted herein, including privacy, publicity, copyright, patent rights, trade secrets, moral rights, or trademark rights.

All permissions and releases granted by me herein shall be effective in perpetuity and throughout the universe, and shall extend to PTC, its successors, affiliates, contractors, licensees, agents, and assigns. I understand that PTC may alter or combine the Pictures, Audio, and Video with other materials, but will not distort any message or point of view that I convey. I waive any right to inspect or approve the final use of the Material.

This Assignment, Permission, and Release shall be construed, governed, and enforced in accordance with the laws of the United States and the State of Hawaii. I irrevocably submit to the exclusive jurisdiction of the federal and state courts located in Hawaii, and waive any objection to jurisdiction or venue in these courts.

I warrant that I am over the age of twenty-one (21) and have the full legal capacity to enter into this agreement. I have read and understand the foregoing.
